Job Title: Support Worker

The Support Worker is a hybrid position offering support to both Second Stage Housing, as well as the Outreach program. The position is responsible for providing support, advocacy, referrals, and resources to clientele by developing a positive relationship, facilitating individual sessions, assisting and empowering clients to recognize their options, assisting clients to mobilize their support networks, and assisting them to utilize their opportunities to move toward healthy and stable futures. The Support Worker is also responsible for assisting with developing and maintaining active outreach programs and ensuring that follow-up services, referrals and practical supports are in place to assist clientele.

General

  • Admit and discharge residents to JPH/Outreach while ensuring all documents/forms are accurately completed. Determine the potential resident’s immediate, short-term and long-term needs and provide support where necessary.
  • Review the expectations of the JPH program with residents and orientate them about JPH programs, policies and procedures where applicable.
  • Actively participate in case management and case conferences as required.
  • Counsel clients individually, in family, or in other small groups regarding plans for meeting needs, and aid client to mobilize inner capacities and resources to improve social and economic functioning.
  • Assist the Outreach team in facilitating “client focused” group sessions based on a pre-determined schedule if necessary to support skills development in the areas of personal accountability, communication, stress management, self-esteem, anger management, emotional regulation, and conflict resolution.
  • Provide support and advocacy to clientele touched by the effects of Family Violence. Provide information, advocate for and refer clients to community resources, human services and other organizations as needed.
  • Regularly review and evaluate resident goal setting and make plan adjustments as appropriate to achieve desired outcomes through weekly one-on-one meetings with residents.
  • Respond to requests for crisis intervention, community information, referrals and general information for residential clients.
  • Transport residents to medical and legal appointments when necessary. Teach the residents about and how to use the Public Transportation System.
  • Participate in duties such as cleaning, laundry, etc. when required. Clean and prepare apartments for incoming residents when required.
  • Ensure JPH residents have the tools, referrals and skills that they will require at the end of the program to live independently.
  • Be an active team member and promote the team “vision.” Be willing to assist/help co-workers when needed and encourage open communication amongst staff by relaying vital information.
  • Assist with training and orientation of new staff and students.
  • Perform other related work as assigned by the Assistant Director and/or designate.

Administration

  • Prepare, complete, and document all necessary forms, support letters, documents, and information required.
  • Document relevant observations/situations in resident file. Establish proper, detail orientated and accurate documentation and resident files.
  • Provide monthly reports.
  • Registered Social Worker (Degree) or post-secondary education in a related field.
  • 2-3 years’ experience and/or working knowledge of the dynamics of family and domestic violence, addictions, suicide and other social issues.
  • Ability to relate to clients in a non-judgmental manner, and to respect client confidentiality.
  • Experience in handling emergency situations, conflict resolution, and crisis intervention.
  • Experience in completing assessments/intake, case planning/management and intervention.
  • Strong organizational and time-management skills and be able to multi-task in a fast-paced environment.
  • Strong written and verbal communication skills, and strong computer skills.
  • The ability to work independently with minimal supervision and in a team environment. Ability to work alone.
  • Clear Criminal Record Check and Child Welfare Check.
  • Standard First Aid/CPR, ASIST Certification, and Non-Violent Crisis Intervention Training is considered an asset.
  • Valid Class 5 Driver’s license free from restrictions

The position is a permanent full-time (40 hours/week) job opportunity, with core hours Monday to Friday, with some evenings, and weekends on an ad hoc basis. The successful candidate must be flexible in working hours to provide coverage as needed.

Compensation rate will be based on assessment of the successful candidate’s education, skills, previous experience and other factors.  The successful candidate will be eligible for employee health benefits, pension contributions as defined by Board policy.
